| Details: | SUMMARY: M/R TCR violation for 3rd quarter 2008, #8909 DETAILS: 10/22/09: A message was left for the operator concerning the missing sample. A request was made to submit the sample if it was taken and not turned in. A letter was also sent outlining these requirements. 10/31/08: Operator called back and said that the 3rd quarter sample was missing. We discussed the need to turn samples in on time and the timeline for doing so. ACTION NEEDED: Submit past sample results is available. Sample every quarter and turn in to the State DW program on time to avoid SNC status. | ||
