Details: SUMMARY: Failing to submit third quarter coliform DETAILS: 10/14/09 Janes phone number is disconnected. I sent email requesting her to call me. Brian Shelton drove to the Bella Rosa and noted that it was closed with a for sale sigh posted. 10/30/09 Jane called and left message for me that the last day of operation was 6/24/09. 11/4/09 I have submitted request to the state to inactivate this system. ACTION NEEDED: None
