| Details: | SUMMARY: Violation investigation DETAILS: Spoke with Sybil and tests were done and inadvertently not sent in. Sybil will instruct her personnel to be sure they submit all test reports. I also reminded Sybil about SDWIS and checking periodically to insure reports are received. I called Umpqua Research to request copies of the tests missed. The system did in fact test for the 2nd and 3rd quarters. I discussed lab reporting and was told by Lisa that they do report tests for clients if requested. This system did request test results to be sent in however the system name of the two quarters in question are entered as South Coast Head Start and not as Upper Chetco School. The results were not sent due to miss identification. Lab assured me that they will mark the file and report in the future. ACTION NEEDED: None | ||
