| Details: | SUMMARY: Failed to Report Coliform 4th quarter 2008 DETAILS: ID 21971: 02/18/2009: Attempted to contact the representative Richard Tkaczyk and left a message 02/24/2009:I contacted Leslie Smith who assists Mr Tkaczyk. I explained the violation to her and asked that she send in the test results to the state DWP right away. She said she will have to contact the lab, but will have them faxed in to state DWP today or tomorrow. 03/03/09: I spoke to Leslie Smith by phone. Leslie said the lab did not take a sample during the 4th quarter of 2008, but did for this quarter (1st 2009). ACTION NEEDED: 04/15/09: Test results for 1st quarter 2009 on SDWIS. |
