Details: SUMMARY: Alert COLI6524 & Violation 3451909 DETAILS: Spoke to Mr. Stevens regarding positive sample. Four repeats were taken and reported, 5 routines were taken in July but not reported (sample results obtained from Umpqua Research). A letter was mailed outlining reporting requirements and explaining the samples may not be reported late. ACTION NEEDED: Monitor for future reporting compliance.
