| Details: | SUMMARY: Alert #9659 DETAILS: After a TC positive on 9/10/08, all 4 repeats taken on 9/12/08 were clear. However, no samples were submitted for October (should have been 5 routines). Then a routine taken on 11/21/08 was TC positive. On 11/28/08, I called Pyxis lab and confirmed that 4 repeats were taken on 11/25/08 and were all clear. On 12/1/08, I called Ed Young to discuss. He said, he thought the bad one in November was due to contamination from a repair of a broken water line. He said, he went back and disinfected the distribution system at that point. He said, he will take 5 routines in December. If all are clear, he will be back to normal monitoring. It appears that they took 10 routines in January instead of 5 in December. But all are okay. ACTION NEEDED: System to take 5 routines in December and call me with results. | ||
