Details:	SUMMARY: Monitoring violation DETAILS: System is having problems getting a clean sample since they had a new vent installed on the system. A water system survey was done on 8/20/09. At that time it was noted they have an unresolved total coliform and needed to issue a public notice. There has been multiple contacts with the system. On 11/16/09 our office was told that they system had been repaired and the vent was not the problem. Contact with the system on 12/2/09 we were told they had contacted Melvin Olsen. Mr. Olsen found the problem to be the vent and had done some work on the system. At that time they were going to chlorinate after the work was completed and take the required samples for December. I placed a phone call to Natasha today asking her to return my call. ACTION NEEDED: Need to take five temporary routine samples and issue a public notice.
