Details:	SUMMARY: Monitoring & Reporting Violation: Coliform DETAILS: Crystalwood PWS notified in writing of violation of coliform monitoring. The system is required to sample for coliforms quarterly. The last sample was taken Sept 19, 2008. The sample results for the fourth quarter(Oct-Dec) 2008 have not been received by the DWP. ACTION NEEDED: The following actions are required to be in compliance: If required samples were taken, copies are to be sent to the State DWP and Klamath County Environmental Health. If samples were not taken: 1) Take coliform for this quarter (Jan-Mar 2009) immediately. 2)For those samples not taken, post Public Notice within 30 days and send a copy to the State DWP and Klamath Co Environmental Healthy within 10 days after issuance.
