| Details: | SUMMARY: System should be Outstanding Performer DETAILS: System operator has requested to be listed as an Outstanding Performer. A survey was recently conducted on 10/6/09 but this was not discussed. No significant deficiencies were noted and the system has only had on violation within the last 3 years(1/2007) and the last positive coliform was 8/2004. Therefore, the system should qualify as an Outstanding Performer and the survey frequency changed to every 5 years. ACTION NEEDED: Outstanding Performer designation will remain in effect as long as criteria continue to be met. | ||
