Details: SUMMARY: Plan Review DETAILS: March 2010 contacted owner/operator regarding notice of nitrate violation. Monitoring requirements discussion lead to lack of well house and well head casing below grade lever. Contacted Evan Hofeld regarding plan review requirements for the well house. 4/13/10 With an onsite visit I facilitated getting the preliminary requested and required information for the state plan review. Pictures of site, storage facilities and well plan review check list, and correction on location of well. 4/16/10 Emailed information to Evan. ACTION NEEDED: Clarification on type of pressure tank from operator. Pending any further action and direction per Evan Hofeld
