| Details: | SUMMARY: Field Consultation for Emergency Response Plan (ERP). DETAILS: Made field visit to operator on 05/03/10 to follow up and discuss content and purpose of ERP. Gave paper to operator ad discussed with them what is required for the ERP and the information is needed. Also gave them information on where the electronic form can be found online. Further discussed the time frame for completing the plan, and where the completion form needs to be sent after the plan is finished. ACTION NEEDED: Operator to complete ERP form and mail completed form to State. County Health Department to follow up with verification of completion on or before 120 day expiration period. | ||
