Details: SUMMARY: Violation ID #1711811 for period August 01, 2010. Thru August 30, 2010 did not submit for Monthly SW Report -Late /Nonreporting - two DETAILS: Violation # 1741811 did not submit monthly SWTR for tine period 08/01/2010 - 08/30/2010. I called the system on 09/30/2010 and spoke with Lowell. Lowell stated that he had already faxed the report the first week of September, but perhaps it was not received. He indicated he would refax the report one more time. ACTION NEEDED: Re-fax August 2010 SWTR.
