| Details: | SUMMARY: Violation - No initial arsenic sample. DETAILS: During survey I notified the operator that State database does not show any initial Arsenic results. The operator provided results tot his office on 05/18/10. I faxed the data DMCE on 0/18/10. This Monitoring Deficiency was documented as a significant deficiency and the survey and had not been investigated as a violation since it was an initial sample that had not been sent to DWP and not a current violation. This corrects all deficiencies front the survey. ACTION NEEDED: N/A | ||
