| Details: | SUMMARY: Follow up deficiencies on sanitary survey DETAILS: Have attempted to follow up with operator regarding the deficiency items in a letter 8/3/10 as well as a telephone call on 8/12/10. I had not heard back either time so today I made a field visit. I did make contact with the operator and we reviewed the items and he again stated he would get the items corrected by this Friday and contact me when all items are corrected. ACTION NEEDED: Additional follow up and correction of significant items as noted in survey | ||
