Details:	SUMMARY: Plan review DETAILS: March 2010 contacted by owner regarding notice of nitrate violation. Monitoring requirements discussion lead to lack of well house and well head casing below grade level. Contacted Evan Hofeld for requirements for well house. 4/13/10 with on site visit I facilitated getting the preliminary requested and required information for state for plan review. Pictures of site, storage facilities and well plan review check list, and correction on location of well. 4/16/10 emailed info to Evan ACTION NEEDED: Clarification on type of pressure tank from operator. Pending any further action and direction per Evan Hofeld.
