Details:	SUMMARY: Change in operator DETAILS: Heidi called 4/11/11 to inform us that her father, David Lentz, owner, passed away. She asked me to mail a copy of the last survey, she knew that there were deficiencies she needed to correct. 4/19/11 she received the survey and doesnt understand what the responsibilities are of the operator. Heidi lives in Illinois. She will call me within the next couple weeks when she comes back to Oregon to set up a time to meet. ACTION NEEDED: none
