Details: SUMMARY: Positive coliform DETAILS: Left voicemail for Luke to call back and repeat samples need to be collected within 124 hours. Spoke to Rebecca at Neilson Research and they have not received repeat sample yet. They have same phone number on their form he filled out when submitting the samples. They always contact person when samples are positive but she doesnt know if someone actually spoke to Luke or left a message. 4/27/11: Called Luke got voicemail. Called Shady Cove Family Dentistry, Luke is a dentist there. Only voicemail. Called Tree of Life Childcare, they are a user of the system, number is disconnected. Called owner of Tree of Life, Amanda OSullivan and it was disconnected. Found number in file, 541-499-2032 and spoke to Amanda. She said the dentist office is open and Luke should be in. I requested she give him a message to contact me ASP regarding the water samples and explained the situation. She said she understood and will give him the message. 4/26/11. Luke called and has already taken repeat samples from 3 different taps and the tap that is closest to the well head. According to Luke, there isnt a tap on the head. When asked about theories he didnt know. He said he is new to the sampling process. I explained to ensure the after runs several minutes, aerators are removed, dont use swivel faucets if possible, etc. as it could cause samples to be positive. I asked if he let the taps run for several minutes prior to taking the repeats and he said he let it run a little while. 5/3/11 Mike Obereigner: The repeat samples collected 4/26/11 negative for coliform. I left a message on Lukes voicemail regarding the 5 temporary routine samples from the distribution system due in May, 2011 ACTION NEEDED: 5 temporary routine samples to be collected in May
