| Details: | SUMMARY: IOC and nitrite violations DETAILS: Email notification of systems non reporting of IOC and nitrites. Called operator and left message. Message back from operator requesting fax of information. Tests were done in January 2011 - per operator. 2/23/11 - Faxed information on violation for nonreporting. Mr. Chamberlain to make sure state gets the lab results. Also requested confirmation that Emergency Response Plan (ERP) has been developed and to send verification to me or the state, per letter Clatsop County Public Health mailed to him in November 2010 reminding him of the requirement.
