| Details: | SUMMARY: Significant deficiencies corrected DETAILS: I have reviewed the Operations & Maintenance Manual (OM) and Emergency Response Plan(ERP) both are acceptable. I provided one recommendation regarding the ERP, provide a padlock for the well head so that any tampering would be evident by a broken lock. ACTION NEEDED: The ERP proof of completion form needs to be submitted to the state. | ||
