| Details: | SUMMARY: Significant deficiency DETAILS: Spoke with Shawn Stevenson about well evaluation and waiver. It was decided that the system could do monitoring for VOCs every 3 years. If theres a detection then we will reevaluate the situation. I called Jim Nash and explained the sampling requirement to him We also discussed the other deficiencies that were noted in the survey. I received a copy that he backflow device had bee tested 5/21/11. ACTION NEEDED: I will follow up on the other deficiencies after the 9/16/11 deadline | ||
