| Details: | SUMMARY: complaint against system DETAILS: Please refer to the attached letter. Please retain a copy of this letter for your records. On going issues against the system, system has failed to correct significant deficiencies as identified during 2/23/10 survey, system has failed to submit arsenic sample, system has failed to maintain timely coliform sampling and missed 3rd quarter 2011 sample. On 11/18/11 this office received a formal complaint against the system. Enforcement action is recommended against this system. ACTION NEEDED: Recommend the state pursue enforcement action against this system. | ||
