Faxed over a blank monthly disinfection report page. Contact time worksheet requires to maintain a 1.0mg/L chlorine residual at all times for 4 log inactivation of viruses. Chuck said he will begin mailing the form to the Portland office. ACTION NEEDED: Submit monthly disinfection report to Portland office.
