Details: SUMMARY: Monthly disinfection report monitoring update DETAILS: Spoke to Jeff and he said he mailed the monthly disinfection report for December 2011 but will put another copy in the mail to the Portland office. I gave Jeff the PO Box 14450 address in Portland to make sure he was sending it to the correct address. ACTION NEEDED: Submit another copy of the monthly disinfection report to Portland office.
