| Details: | SUMMARY: Follow up to monitoring violation DETAILS: Contacted Theresa to ask if she mislabeled her lab slip for 4th quarter and she said she must have. They did take a sample on 12/9/11 but labeled it an AS and at SRC. State website states it was taken at the kitchen sink. I emailed Chuck and asked him to change it to a RT in distribution and to remove the violation. I checked the website and it was changed and the violation was removed. ACTION NEEDED: none | ||
