| Details: | SUMMARY: Deficiency follow up, corrective action plan DETAILS: Deficiencies from April 13, 2012 survey. Well does not meet setbacks from hazards. A portable toilet was sitting 20 from the well. 7/21/11 I drove by and the portable toilet has been removed. This deficiency has been corrected. I addition, the new well was within 100 feet of the old well that had not been properly abandoned. Discussed with operator on the phone on 1/25/12 and the well still hadnt been properly abandoned. The operator has not been able to locate the old well. This deficiency has not been corrected. I explained that I was putting him on a Corrective Action Plan and we agreed he would have the deficiency corrected by May 1, 2012 ACTION NEEDED: none | ||
