| Details: | SUMMARY: Significant deficiencies corrected DETAILS: 10/18/11 - I reviewed the Operations and Maintenance (O&M) manual, emergency Response Plan (ERP) and the flood Contingency Plan. All acceptable with recommendations as follows ERP - add notify health dept if the system was thought to be compromised-- O&M manual - Specify ratio of water to chlorine-- Flood Contingency Plan - specify location of circuit breakers and specify how to turn well pumps off (breaker, valves, etc.) ACTION NEEDED: The ERP proof of completion form needs to be submitted to the state. | ||
