| Details: | SUMMARY: Field visit for deficiency corrections DETAILS: Significant deficiencies noted during 9/20/11 survey. Crack in concrete slab, open port in top of well head and no protective housing. Made field visit and spoke with Jerry. A new concrete slab has been poured and a wooden insulated dog house with metal roof has been built. The open wire port in the top of the seal has been filled. ACTION NEEDED: Continued routine monitoring and maintenance. | ||
