| Details: | SUMMARY: Survey deficiency follow up DETAILS: I talked with operator about meeting the 18 week survey deficiency deadline. He said he completed the coliform sampling plan and that the operations and maintenance manual (OM) would be done next week. All information was received in the office on April 5th. Deficiencies found on survey done on 11/29/11 are as follows: No written coliform sampling plan and no written OM manual. All deficiencies/rule violations have all been corrected. ACTION NEEDED: none | ||
