Details: SUMMARY: Sanitary survey follow up DETAILS: Deficiencies noted on sanitary survey: Emergency response plan was not completed. A public notice was not issued as required for the violations in 2008 and 2009. A CCR was not submitted to users and certified with the state every year. The Direct Responsible Charge (DRC) does not have written operating protocols. All deficiencies were corrected. ACTION NEEDED: none
