| Details: | SUMMARY: Survey Significant Deficiency follow up DETAILS: Mr. Pfaff in the office to submit documents requested during 9/16/10 survey. Required documents included Emergency Response Plan (ERP), Enabling Authority (EA) and Annual summary report (ASR) All of these items were submitted and I reviewed the ERP and it looked complete. The ASR and EA had already been sent to the state. (missing plan review for new well was submitted and approved on 2/15/12 ACTION NEEDED: Continued annual ASR and updating all written plans as needed. | ||
