Details: SUMMARY: Follow up on Significant Deficiencies (SD) DETAILS: Met with Chuck in field 5/31/12 to review the list of items identified during the sanitary survey on 2/14/12. An operations and maintenance manual has now been completed. This system has corrected all the significant deficiencies as identified during the 2/14/12 survey ACTION NEEDED: none
