| Details: | SUMMARY: Failed to submit April 2012 source assessment sample from SRC-AA DETAILS: Violation was assessed for failing to submit an April 2012 source assessment sample from SRC-AA. I spoke with Sunny and she faxed new copies if assessment samples from SRC-AA from May, July , August of 2011 that were mislabeled specials. I spoke with Chuck Michael who informed me these would be accepted and the 12 month assessment schedule would be closed out. I sent the state the sample result via email on 6/6/12 ACTION NEEDED: None | ||
