Details: SUMMARY: Water System Survey 3/5/12 DETAILS: Deficiencies: The cross connection program requires a system to submit an annual summary report. The following is a link to the required report. Submitted report to state 3/22/12. The system does not have an operation and maintenance manual and I have enclosed an outline. System completed O and Ma document during onsite visit. System had request ion an onsite visit since the O and M document was a new tool. Stephanies husband had been in charge of the paperwork and died suddenly. ACTION NEEDED: None
