| Details: | SUMMARY: Alert - two of five source confirmation samples tested E coli positive (all 5 tested TC+) DETAILS: 5/28/12 Two of five source confirmation samples tested E Coli positive (all 5 tested TC+). I emailed Gary a Tier 1 boil water notice and made a site visit. Gary will be shock chlorinating the system this weekend. 7/2/12 As per policy, I filled out and sent a source evaluation request form to the DWP. I spoke with Gary and informed him that I would let him know when I find out the evaluation results and the required corrective actions. I advised him that the boil water notice needs to stand until corrective action is completed. I added that five temp routines from the distribution system need to be collected sometime this month, but not until the chlorine is back to normal. 7/5/12 Five temporary routines were collected – all tested coliform absent. 7/11/12 letter sent to Medford Moose Lodge outlining the DWP alternatives for corrective actions. 8/6/12 Gary Gentry stopped into the office to inform me Medford Moose Lodge will be drilling a new well. I gave him Scott Curry’s phone number to answer questions regarding the plan review process. ACTION NEEDED: System needs to proceed with Plan Review application for the proposed new well. | ||
