| Details: | SUMMARY: Violation 903396527-was incurred as a result of a failure to maintain 4 log treatment DETAILS: 6/29/12 Spoke with Carol and she informed me the report sent to the DWP contained decimal errors and that there was no failure to maintain 4 log treatment. She will be faxing an amended report to DMCE. I spoke with Chuck Michael and he will review the report and get back to me. 8/8/12 Violation has been removed. ACTION NEEDED: No further action needed. | ||
