Details:	SUMMARY: Follow up for Deficiencies from 4/12/12 WSS DETAILS: Deficiency follow up for items on 4/12/12 survey-- Plan review for new well not completed, No written coliform sampling plan, Wire port opening in Well #1. ** All deficiencies have been corrected. John Mason site visit verified opening in wellhead sealed with steel wool and system submitted a written coliform plan. Plan review 182-2011 completed 6/1/12. ACTION NEEDED: All deficiencies noted during survey are considered corrected.
