Details: | SUMMARY: Violation - Source water sample not submitted on time/Public Notice not issued. DETAILS: Emailed Mike regarding the monthly Source Sample late from well (Lane 18477) for the positive temporary repeat samples in May 2012-- The repeat samples are required to be submitted within 24 hours or NO LATER than 2 weeks from the positive sample. The repeat samples were submitted 20 days after the positive sample resulting in a violation. The SDWIS data base is an excellent tool to use to keep tract of when your Coliform samples are due and where to take them if you ever have questions, www.oregon.gov/dhs/ph/dwp click on drinking water online. You can pull up your water system by Water System (WS) name of WS ID lookup (91897). The following guideline can be found under Coliform Sampling schedule link located at the bottom of your page- NOTE the bold highlighted sample required for a ground water source for one of the repeats to be taken by 6/9/12. Crow High School SD66 PWS 91897–Dist-a GWR Triggered sample(s) to be taken 5/25/2012-06/09/2012 at SRC-AA Well.The following schedule shows the samples submitted. A violation occurred because the source groundwater TG sample was submitted late – on 6/15, should have been in by the 9th. The total system score for Crow High School is at 19 right now. The State Office considered this level to be at an enforcement action level requiring action steps to be taken to fix the issues. Please make sure to have your Coliform Sampling plan ready and easily available for all employees to assist with repeat samples. If for some reason you are unavailable to help out. NOTE the public notice late non reporting–A PUBLIC NOTICE is required to be posted EVERY time a repeat Coliform sample is confirmed. My office must receive a copy to send to the State.
