Details: SUMMARY: Violation - Failed to report TC 2012 QTR 2012 DETAILS: I contacted Katsu who is assisting the operator. I explainied the violation to her. She thought the lab sent in the test results and they did have a sample taken in 2nd QTR. She will fax on the test results. ACTION NEEDED: The test result is on SDWIS - reported late.
