| Details: | SUMMARY: Follow up on survey conducted in 2011 DETAILS: Spoke with Dans wife about attempts to follow-up on the survey from 2011. Dan was currently at work. She provided me with his cell phone number. I called the number and left a message about the survey follow-up and to please return my call. To date 10/16/12 there has been no return phone call. ACTION NEEDED: Contact Douglas County and provide documentation of Cross-Connection Control Program, designation of enabling authority, CCR, Emergency Response Plan and Annual Report. | ||
