| Details: | SUMMARY: Survey deficiency follow up DETAILS: Water System Survey deficiencies included the following: 1)no cross-connection control program. 2) no completed ERP. 3) no CCR. Met with the operator on-site on 7/31/12 and gave him 2 weeks to complete and resolve all deficiencies. Gave him copies of the ERP and CCR templates/aids and cross connection information. Follow-up e-mail sent 8/9/12, no reply. Checked in via phone 8/16/12 and gave one more week to get things done. Left message 8/23. Left message 9/11. No corrections verified as of 9/11/12.
