| Details: | SUMMARY: Field Consulation for Emergency Response Plan (ERP) DETAILS: Made field visit to operator on 5/2/12 to follow up and discuss content and purpose of ERP. Worked together with operator to complete 95% of the ERP, operator had to get some additional names and contact information for ERP to be totally complete. ACTION NEEDED: Operator to complete ERP form and mail completion form to state. County Health Department to follow up with verification of completion on or before 120 day expiration period. | ||
