Details:	SUMMARY: Deficiency follow-up from WSS 5/14/12 DETAILS: The significant deficiencies and rule violations noted are as follows-- 1. The system does not have Maintenance and Operations, completed 9/5/122. The Sanitary seal on the well is no longer water tight and must be replaced – Replaced 6/18/12 and viewed 8/23/12.3. There is no Coliform Sampling Plan – I have included a template for such a plan – Completed 8/23/12 ACTION NEEDED: No further action, the sanitary seal has been repaired as of 6/18/12.
