Details: SUMMARY: Significant deficiencies DETAILS: Thirty days have passed since the system survey report was sent to the operator. Our office has not been consulted about correcting the significant deficiencies noted on the survey. I sent a letter to the operator reminding him of the 18 week correction time frame. ACTION NEEDED: Need to be corrected by 2/6/13
