| Details: | SUMMARY: System survey deficiency follow up DETAILS: System survey 11/14/12. Both deficiencies identified during the survey have been corrected. The ERP has been completed by the system. A copy of the CSP and ERP completion verification form was faxed to me on 12/4/12. I emailed a copy of the ERP completion form to the state. ACTION NEEDED: none | ||
