| Details: | SUMMARY: Significant deficiencies (SD) corrected DETAILS: All SD from the 10-3-12 survey have been corrected. I have reviewed the O&M manual (recommended a few alterations) the ERP and the chlorine residual log. I noted the backfl0w device had been tested and that NSF approved sodium hypo chlorite is in use.
