| Details: | SUMMARY: Significant deficiency follow up DETAILS: Deficiency follow up from June 6, 2012 system survey. Gap in well head seal. Spoke with operator John and steel wool has been packed into gap in space between well head cap and casing to preclude contaminants. This will be permanently addressed next time work is done on the wellhead. John submitted photos of correction. ACTION NEEDED: Routinely check the condition of wellhead/seal and make sure not openings. | ||
