| Details: | SUMMARY: Survey deficiency follow up DETAILS: 9/18/12 survey showed two administrative significant deficiencies: No enabling authority submitted and no CCR. Received both documents from Heather and Dave Wayland 11/20/12. Received CCR and certification form. CCR was distributed but not sent to the state. Will submit copy of EA to state. ACTION NEEDED: Continue to distribute CCR each year by July 1 and get copy to the state and submit annual summary report. | ||
