Details: SUMMARY: Deficiency follow up DETAILS: Survey date 9/6/12. No flap valve, screen or equivalent on reservoir drain. On October 5, system provided pictures of the screened overflow drain. This deficiency has been corrected. No Operation & Maintenance Manual. System provided a complete copy on January 23, 2013. All deficiencies have been corrected. ACTION NEEDED: none
