| Details: | SUMMARY: System survey 30 day follow up DETAILS: Beth contacted my office by phone j/2/13 to let me know they are correcting the significant deficiencies cited on the November 15, 2012 survey. On January 9 I visited system to speak with them in person. The following has been corrected: Protective screen was installed on well #4. They were able to locate their operations and maintenance manual for well #4. They provided me with a copy of their 2011 Cross Connection ASR ACTION NEEDED: Continue writing out the coliform sample plan | ||
