| Details: | SUMMARY: Complete/corrected survey deficiency follow-up DETAILS: A significant deficiency was cited in the survey on August 14th for not meeting setbacks. Four options for correction were provided in the cover letter. The owner/operator Colleen Riley requested an evaluation for a construction setback waiver. This was approved 9/18/12. A letter was sent to the Ms. Riley explaining the waive approval, copy of letter provided with this contact. ACTION NEEDED: continue to monitor coliform and nitrate testing. | ||
